We will work with you and your insurance company to ensure that you get the greatest possible benefit from your plan. It is important that you notify us of any changes in your plan or your address. Your insurance policy is a contract between you, your employer and the insurance company. Many insurance companies will not provide us with information directly.

At the time of treatment, you will be responsible for any portion not covered. The office does accept Visa, Mastercard, American Express, bank debit cards and cash.
